Ocular findings in haemochromatosis.

چکیده

HAEMOCHROMATOSIS is a rare condition, the chief clinical features of which are pigmentation of the skin, cirrhosis of the liver, diabetes mellitus, and hypogonadism. The opportunity of examining the eyes of five patients in whom the diagnosis had been confirmed by liver biopsy arose through the kindness of my colleague, Dr. N. F. Coghill, who is incorporating the clinical and pathological investigations in a forthcoming publication. Detailed clinical histories have not, therefore, been included in the present communication. A review of the literature since 1930 has revealed very few references to the ocular findings. Maddox (1933) describes the retinal appearances ob-, served in four cases, and this reference is quoted by Duke-Elder (1940). Lawrence (1949), in his Banting Memorial lecture, commented upon the absence of vascular changes in the pure and primary forms of islet and pancreatic destruction, and contrasted these with appearances found in primary diabetes; he suggested that a common factor, perhaps the pituitary, might be responsible for both the vascular degeneration and the diabetes, and then referred to two casereports (relapsing pancreatitisSprague, 1947; haemochromatosis -Stawell, personal communication), in which the retinal chan-ges of diabetes were observed.

Deposition of melanin and iron in ocular structures in haemochromatosis

Cutaneous pigmentation is a well-recognized feature of idiopathic haemochromatosis and, in various series, has been recorded in up to 8o per cent. of patients (Finch and Finch, I955). In the world literature there are occasional reports of pigmentation in the gums (Saundby, I890), buccal mucous membranes (Richardiere, I895), lips (Parker, I903), and tongue (Hess and Zurhelle, I905), and Maddox ...

SMALL INTESTINE Duodenal expression of iron transport molecules in untreated haemochromatosis subjects

Background and aims: In HFE associated hereditary haemochromatosis, the duodenal enterocyte behaves as if iron deficient and previous reports have shown increased duodenal expression of divalent metal transporter 1 (DMT1) and iron regulated gene 1 (Ireg1) in affected subjects. In those studies, many patients had undergone venesection, which is a potent stimulus of iron absorption. Our study inv...

Haemochromatosis: unexplained metacarpophalangeal or ankle arthropathy should prompt diagnostic tests: findings from two UK observational cohort studies.

OBJECTIVES To examine demographic and clinical features leading to the diagnosis of hereditary haemochromatosis and assess factors that might enhance earlier diagnosis, with particular attention to arthritic symptoms. METHOD Diagnostic features were captured directly from patients with haemochromatosis attending a specialist rheumatology clinic (group 1) and from analysis of a specifically de...
